Espresso machine interior and exterior was observed to have food residue and was not clean to sight and touch. Follow manufacturers cleaning instructions for cleaning and sanitizing. Freedom Village @ Brandywine, 15 Freedom Blvd. Coatesville, Follow-up, January 5. Pass. Leaking Trash Compactor. The compactor has been inspected and either needs replaced or a new floor panel. Correct within 14 days. The following floorings were observed to be in disrepair: Main kitchen dishwasher area, ripped flooring near the floor drains and in dishwasher area - clean side drain board; Health Care Center - Rehab Kitchenette, missing section vinyl plank flooring. Repair or replace flooring. Ensure floors are maintained in good repair. Smooth, durable and easily cleanable. Management has stated the floorings will be repair/patched.

Great Valley Senior High School, 225 N. Phoenixville Pike, Malvern, January 5. Pass. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in The Traulsen single-door refrigerator next to the pizza warmers. Provide a thermometer immediately. Temperature measuring device inside Traulsen refrigerator next to pizza hot holders observed to be a hot holding thermometer. Replace with a regular ambient air thermometer that can measure temperatures 41°F and below.

Kathryn D. Markley Elementary School, 354 Swedesford Rd. Frazer, January 5. Pass. In the walk-in refrigerator, observed ceiling fan grates to have an accumulation of dust. Clean immediately to prevent potential contamination of food. A chemical spray bottle of bleach mixed with water was observed without a label. Person in charge labeled the chemical bottle.

Manor Farm Market, 1410 Manor Rd. Coatesville, January 5. Pass. Operator had products (bakery and jarred items) for sale from unapproved sources. Baked goods were produced from the farm residence (unlicensed). Jarred items (vegetables, sauerkraut) came from a neighbors private residence (unlicensed). It is a responsibility of an operator to ensure all foods come from an approved source. Maintain validation (invoices, copies of licenses, inspection reports) on site. Ensure all products come from valid approved sources. Pickled Beets, Jarred fruits, Jams/Jellies, Applebutter, Sauerkraut and cupcakes were produced from unapproved/licensed sources. Items were removed from the store front at the time of inspection. May not sell food from an unapproved source. Owner shall validate all foods being re-sold are from an approved source. At the time of inspection all items from unapproved sources were removed from the shelving. Products may not offered in commerce.

Oxford Presbyterian Church, 6 Pine St. Oxford, January 5. Pass. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was <50ppm, and not 50-100 ppm as required. Sanitizer was primed during the inspection- observed 50ppm.

Royal Farms #225, 5031 Horseshoe Pike, Downingtown, Follow-up, January 5. Pass. No violations.

The Hungry Pilot, 1 Earhart Dr. Suite 3, Coatesville, Opening, January 5. Pass. Bar glasswasher and kitchen dishwasher were not dispensing chlorine at 50-100 ppm as required. These pieces of equipment are new. Repair service was called during the inspection. All equipment placed in dishwasher/glasswasher must be manually sanitized until properly dispensing chlorine. Correct immediately.

The Record Kitchen & Bar, 204 E. Lincoln Highway, Coatesville, Follow-up, January 5. Pass. Seal prewash sink in kitchen to wall behind and seal plumbing in bar area passing through the floor. Correct within 7 days.

The Social @ Downingtown, 541 W. Lancaster Ave. Downingtown, Follow-up, January 5. Pass. Plastic cups were observed in the Dry Storage Bins for dispensing food instead of scoops with handles. Must use scoops stored with the handles up in these containers. CORRECT WITHIN 24 HOURS. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Provide an unexpired Chlorine Kit for Kitchen and QT 10 kits for use at the Kitchen and Bar. Correct within 24 hours. Kitchen: The quaternary ammonium concentration in the sanitizing solution of the 3-bay ware wash sink was 0 ppm, rather than 200 ppm as stated on the manufacturers use directions. Employees were not using the correct amount of water to Quat Tablets. This was explained and shown on site. The level was then found at ppm. INFORM ALL EMPLOYEES OF CORRECT AMOUNTS AND POST DIRECTIONS TODAY. The following still need cleaned: Wall under the cooking exhaust hood-remove grease from all piping; 1-Door Dukers Freezer-bottom. Correct the above within 24 hours. The hand wash sink on the cooking line was once again observed blocked by a cart full of cooking supplies and not accessible at all times for employee use. The sink was also full of utensils. Cart was moved and sink emptied. Must rearrange items in the kitchen so there is not a need for this cart. Correct today. Repair the hole in the ceiling over the food prep area Kitchen to be smooth and cleanable. Clean the ceiling and light shields in the Kitchen prep area. Clean the floor under the convection oven. Clean the floor drain under the food prep sink Correct within 48 hours. 

The Social Lounge, 117 E. Gay St. West Chester, January 5. Fail. In the front bar area, observed metal cup being used as an ice scoop. Replace the cup with a scoop with a handle to reduce the potential of contamination of the ice. Grilled chicken was held at 52°F, in the grill area, rather than 41°F or below as required. Person in charge explained chicken was put there an hour prior to inspection to be put in the prep top but forgot. Chicken was put in the walk-in and the temperature was reduced to 41°F by the end of inspection. Observed multiple time/temperature controlled for safety (TCS) ready-to-eat foods without a datemark of when they were prepared under the prep top coolers. Ensure all TCS RTE foods kept for longer than 24 hours are marked with the date they are prepared. Observed the sliding door of the True refrigerator next to the deep fryer in the kitchen to be in disrepair. Fix the door so that it is tightfitting and closes seamlessly. The hand sinks at each end of the prep line were blocked by trashcans and not easily accessible at all times. Person in charge moved the trashcans during inspection. Observed the door in the back of the kitchen leading outside to have a gap in the bottom of the door which could be a potential entryway for pests. Fix immediately.

Carmine's New Pizza & Eatery, 947 Paoli Pike, West Chester, January 4. Fail. A food employee was observed touching and slicing cooked pizza with his bare hands, which is prohibited due to contamination risks. Use gloves and/or utensils to prevent barehand contact with ready to eat foods. On top of bain marie, label the squeeze bottles with the common food name. Wooden salad bowls found on drying rack. This material is not permitted for use as a food contact surface. Discard bowls. Trash bag found in use to line bread drawer. This type of bag is not permitted for use for direct food contact. Use food grade bags/liners for unwrapped foods. Provide thermometers in the two upright freezers. Replace the missing cove base tile near the pass-thru front counter area and the beverage display refrigerator.

Casa Herrera, 2755 E. Lincoln Highway, Thorndale, January 4. Pass. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the walk in refrigerator, are not being date marked. This was corrected on site. Comply everyday. Supply a new chlorine test kit-your current ones expired in 2017-and supply a QT 10 kit for use at the Bar. Correct within 7 days. The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 50-100 ppm as required. Items must be washed, rinsed and then sanitized. This was explained and corrected on site. Sanitizer was found at 50-100 ppm. Comply everyday. Please submit a copy of ServSafe Certificate, the CFM Application and fee within 7 days.

China Moon, 1247 Horseshoe Pike, Downingtown, January 4. Pass. Customer Accessible Pepsi cooler - Staff are storing bubble tea products in this cooler. Because this is customer accessed only manufacturer prepackaged foods/beverages may be kept in it. Remove the bubble tea products and keep in a refrigeration unit accessible by staff only. Observed food packages stored on the floor panels of both the walk-in cooler and freezer. Keep product stored on approved shelving or dunnage racks a minimal of 6 inches off the floor base. Low mein observed cooling at room temperature 76°F. Homemade hot/sweet sauce observed cooling at room temperature. Provide assisted means of cooling TCS foods down in a rapid manor. Product must be cooled rapidly down from 135 to 70°F within 2 hours, and then down to 41°F within an additional 4 hours. Staff are to monitor cooldown temperatures to ensure safe practices are met. Cooling down at room temperature is not a recognized practice. Cool down in shall layers in refrigeration, using an ice bath, ice stirrer bar or other approved assisted means. Product relocated to the walk-in cooler at the time of inspection. Shredded lettuce was observed stored at room temperature. Shredded lettuce is a TCS food and must stored at safe temperature (refrigeratred below 41°F). Lettuce had been placed out within the hour. Lettuce was relocated to the bain marie at the time of inspection. Keep shredded lettuce stored in refrigeration at a temperature below 41°F. Fish observed thawing at room temperature in reduced oxygen or vacuum packaging. These packages must be opened to allow oxygen to enter prior to beginning the thawing process. Product opened at the time of inspection. Use one of the following methods to thaw product - under cold running water, in refrigeration or as part of the cooking process. Ensure all fish is removed from ROP packing previous to starting the thaw process. Only commercial grade food grade equipment may be used in operation. Replace/remove the following: Black and decker toaster oven; Rubbermaid bins using for keeping rice/flour. Replace within food grade containers within the year. Clean the following: Egg roll racks above the food preparation sink; soiled. Clean and relocate drain rack at the food preparation sink. Bubble tea and smoothing mixing containers must be cleaned using the three compartment sink. Clean and relocate. Soy Sauce containers were stocked in front of the rear handwashing sink. Keep the handwashing sink unblocked and accessible at all times. Relocate soy sauce containers. Clean the following structure: Wall paneling near the three compartment sink and rear handwashing station soiled; Wall paneling near the food preparation sink, splatter; Wall paneling near mop sink, soiled; Flooring below the deep fryers, oil. Clean all within 24 hours. Maintain clean to the sight and touch.

Dunkin Donuts/East Goshen, 1500 Paoli Pike, West Chester, January 4. Fail. Large ice machine interior, a food contact surface, was observed to have food residue and was not clean to sight and touch. Clean and sanitize today. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following: Fan covers inside undercounter Delfield coolers; Inside especially along the top of the Silver King cold holding unit holding hashbrowns. Ceiling tiles missing in the service window/food preparation and needs to be replaced. Replace ceiling tile.

Hissho Sushi @ Giant #6105, 350 Scarlett Rd. Kennett Square, January 4. Pass. Sushi rice pH was not checked before sushi was made. Log data for today was blank. Operator began making rice at 6:30 a.m. and began making sushi at 8:30 a.m. No information had been recorded. The pH check was done on remaining rice and was 4.1. Log data was recorded. Ensure pH is checked and recorded before rice is used to make sushi. No Chester County Food Manager certificate is posted. 

Kentucky Fried Chicken/Taco Bell/West Chester, 519 E. Market St. West Chester, January 4. Pass. Caulk around three-compartment sink and mop sink in chicken cooler was damaged. Reseal sinks within 14 days. A few shelves in the walk-in refrigerator are rusty. Replace shelves within 14 days. Do not paint. Baking sheet pans are beginning to accumulate encrusted food residue. Clean or replace within 5 days. Blodgett oven was observed with an accumulation of oil residue. Clean or remove (if not used) oven within 5 days. Chicken rack in chicken walk-in cooler had a build-up of food residue. Clean within 3 days. Lids for white dry storage containers near cooking line had a build-up of oil residue. Clean within 3 days. Shelves in walk-in refrigerator are in need of cleaning. Clean within 5 days. Sides of cooking equipment have a build-up of oil residue. Thoroughly clean equipment within 5 days. The following floors and/or walls were observed dirty and in need of cleaning: Floor in walk-in freezer under shelves; Ceiling vents and ceiling tiles next to vents in kitchen area; Floor in chicken walk-in cooler below chicken rack; Floor drain below three-compartment sink; Wall above three-compartment sink; Floor and wall behind cooking equipment. Clean areas within 5 days.

Liliana's Pizza & Grill, 1160 Newark Rd. Toughkenamon, January 4. Pass. Knives observed stored between sandwich unit and table. Do not store knives between equipment. Tuna salad, pasta and desserts were not date marked. All foods that are kept for more than 24 hours must be date marked with the date made. Desserts must be dated with date thawed. Replace the torn gaskets on the 2 door True refrigerator in kitchen. The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was over ppm, rather than 50-100 ppm as required. The sink was drained and refilled to proper concentration. No more than 2 cap fulls of bleach are needed in the sanitizer bay. Train staff on proper sink set up.

Limestone Pizza & Grille, 3161 Limestone Rd. Cochranville, January 4. Fail. Label dressings in the bain marie. Sauces and meatballs observed in hot holding at temperatures of 60-61°F. These must be rapidly reheated to 135°F (if commercially processed foods) or 165°F (foods prepared in facility) prior to placing in the unit. Ensure hot hold unit is on and at proper temperature prior to placing in unit. Foods were pulled and reheated on-site. Maintain. Foods prepared in facility, then frozen need to be date marked with prepared-on date, frozen date then thaw date to ensure proper rotation and use within 7 days of preparation when stored under refrigeration.

Market Street Grille, 6 W. Market St. West Chester, January 4. Fail. Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between. Employee did change gloves and properly wash hands after violation was mentioned. Make sure all employees change gloves after changing tasks and wash hands when changing gloves and in between changing tasks. An unopened bottle of Salted Caramel Syrup was observed stored in a crate with cleaning equipment. This was removed immediately. Do not store any packaged food with clean equipment. A cup was observed being used as a scoop for sugar in the basement. A scoop with a handle must be used to prevent hand contact with sugar. Correct immediately. Observed soda syrup boxes and dry storage under a waste line in the basement. Relocate soda syrup boxes and dry storage items within 3 days. It was reported that food was placed in the steam table without reheating food first and a couple food internal temperatures were not reaching 135°F as required for hot holding. All food was discarded during the inspection. Remember to reheat food to 165°F or above and maintain a hot holding temperature of 135°F. French onion soup and Chili on steam table were observed below 135°F as required. This was discarded during the inspection. Remember to maintain hot holding temperatures at 135°F or above. A few ready-to-eat food items were observed stored in the refrigerator located near cook's line were observed without dates. Make sure all ready-to-eat items kept for more than 24 hours are dated, including sliced tomatoes. Correct procedures immediately. A wood crate was observed below the Avancto refrigerator located in front service area. Remove crate within 5 days. Cooks did not have a probe thermometer for checking internal food temperatures. Provide a thermometer within 24 hours. Small Magic Chef refrigerator did not have a thermometer to measure the ambient air temperature. Provide a thermometer within 24 hours. Cooking exhaust hood is due for cleaning. Have hook professionally cleaned within 21 days. Equipment was operating outside of cooking exhaust hood creating grease accumulation on the outside of the hood. Move equipment as to allow the cooking exhaust hood to overhang all equipment by 6 inches. Correct within 24 hours. Chest freezers in basement are beginning to create ice build-up. Defrost freezers within 7 days. Clean knives were observed stored on the table below the electric slicer. The table was observed dirty with food residue. Knives were placed in the dishwasher for cleaning. Do not store under slicer. The electric slicer, was observed to have some food residue remaining after slicer was cleaned. This was completely disassembled and cleaned during the inspection. Remember to thoroughly clean the slicer after each use or a minimum of every four hours including table below. Slicer must be disassembled for thorough cleaning. Clean side of cooking exhaust hood where equipment was not properly under the hood. Correct within 5 days. Clean dry storage shelves located near cooking line. Correct within 5 days. The floor tiles near the soda fountain machine and floor tiles below the cooking equipment were observed damaged. Repair floor within 21 days. Walk-in refrigerator floor located in the basement was observed damaged on the outside at the entrance. Repair within 30 days. Outside area near grease drum was clutter with equipment and food debris. This was addressed during the inspection. Make sure outside areas are kept clean. Small flat top grill was observed being used in the back food prep area near dishwasher. This area does not have a cooking exhaust hood and grease was observed beginning to accumulate on ceiling. Remove flat top grill within 48 hours.

McDonald's/Tredyffrin (Doane), 260 W. Swedesford Rd. Berwyn, January 4. Pass. The following equipment not observed in good repair: Excessive ice build up observed in the walk in freezer, including walls, ceilings, and condensation line under compressor. Have unit serviced and send corrective action photo to the Department. Mechanical low temperature, ware washing equipment observed with buildup inside. Contact Eco Lab and have unit De-Limed at ceiling and side edges. Plumbing system not maintained in good repair; 1 spray nozzle missing at three bay sink. Anise indicated a replacement has been ordered. Broken equipment, old fryer line, observed in dumpster area outside of the food facility needs removed.

Paoli Pike Sunoco (Kavss Inc.), 1425 Paoli Pike, West Chester, January 4. Fail. Chicken roll was held at 127°F, in the hot rollers, rather than 135°F or above as required. Temperature was adjusted during the inspection. Food were held at 44°F, in the True merchandiser cooler, rather than 41°F or below as required. Adjust to maintain foods at 41°F or below as required. Commercially processed ready to eat food, and held more than 24 hours, is being date marked with a discard by date exceeding 7 days from thaw date. Provide manufacturers determination. Food contact surfaces are not being properly washed, rinsed, and sanitized every four hours. Facility is using alcohol wipes instead of following proper warewashing procedure. Re-train staff. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the interior of the True sliding doors cooler. Old unused equipment stored in warewashing area and behind the counter must be removed from food facility. Food facility is using a bleach that is not approved in the Code of Federal Regulations for use in a food facility. Bleach was replaced with an approved bleach during the inspection. The chlorine concentration in the sanitizing solution spray bottle was greater than 200ppm, rather than 50-100 ppm as required. Solution was discarded during the inspection. New solution measured at 100ppm.

Pat's Select Pizza & Grill, 351 N. Third St. Oxford, Change of Owner, January 4. Fail. No datemarking was observed for the prepackaged grab & go desserts located in the Coke refrigerator. All prepackaged grab & go desserts must be date marked when removed from the freezer to the refrigerator for customer self-service. Desserts must be used or discarded within 7 days. The desssers were dated during the inspection; ensure consistent datemarking. No labels were provided for the prepackaged grab & go desserts located in the Coke refrigerator. Provide proper labels containing the following information: Name of product; Name & address of manufacturer; Full ingredient list including any sub-ingredients; Seperate allergen declaration. Fish observed thawing in reduced oxygen or vacuum packaging. These packages must be opened to allow oxygen to enter prior to beginning the thawing process. Fish thawing in original ROP packaging was discarded. Reviewed proper thawing of ROP foods with CFM; ensure proper thawing of ROP fish or foods. The hot water faucet at the hand sink to the right of the dishwasher is not working properly. Make necessary repairs. It should be noted that there is another hand sink easily accessible. The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was less than 200ppm, rather than 200ppm-400ppm as stated on the manufacturers use directions. Additional sanitizing tablets were added- observed 200ppm. Ensure proper sanitizing solution concentration between 200ppm-400ppm. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer.

Pizza Hut #4965, 520 E. Lancaster Ave. Downingtown, Opening, January 4. Pass. The dumpster is to arrive this afternoon. Plan to open Jan. 18. Must email a picture of the dumpster once it arrives. Cannot open without a dumpster. Must obtain a Chester County Certified Food Manager. Please submit the CFM Application - left on site - and the Fee within 7 days.

Pope John Paul II Regional Catholic Elementary School, 2875 Manor Rd. Coatesville, January 4. Pass. The walk-in freezer was in need of repair: Door latch not properly seated, allowing to lock; Door gasket worn/ripped; Ice accumulating on floor due to a possible insulation leak from sealing panel joint. Have the unit serviced and repaired.

Quality Inn Hotel & Suites, 943 S. High St. West Chester, January 4. Pass. In the front lobby (on the wall by the CCHD retail food license), post the following documents in public view: Chester County Certified Food Manager Certificate; Inspection report availability signage.

RSD 81 Baltimore Pike Donuts LLC, 81 Baltimore Pike, Avondale, January 4. Fail. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Ice scoop in portable ice bin observed stored in the ice and not with handle above the top of the ice. Foods in sandwich prep bain marie not date marked. At the bulk sugar container, the handle for the scoop is broken and it is cracked. Replace. Sanitizer test strips are water damaged. Replace. An accumulation of ice observed in the walk-in freezer. Remove. Make all necessary repairs to prevent accumulation. Replace gaskets at sandwich prep station drawers. Replace gaskets at the 1-door under counter refrigerator at the hot chocolate dispenser. Review of warewashing process found that dishes are not being sanitized. Dishes must be washed, rinsed and sanitized. Smallwares including pitchers at cappuccino machine not being washed, rinsed and sanitized at least every four hours. This was reviewed with staff on-site. The floor sink at the 3-compartment sink is slow draining. Repair.

RSD 321 N. 3rd St. Donuts LLC, 321 N. Third St. Oxford, January 4. Fail. Observed cell phone store on the cutting board/food preparation surface at the bain marie. The cell phone was removed during the inspection- clean and sanitize this surface. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration at the 3 bay sink. Test strips provided were severely water logged and no longer usable. Sample pouch of 15 test strips provided for sanitizer in use. Obtain appropriate test kit within 10 days. Bain Marie Cutting Board- food preparation surface: Observed deeply scored cutting board not resurfaced or discarded as required. The cutting board is no longer able to be cleaned effectively. Replace cutting board. Observed mold-like growth on the interior ceiling of the ice machine above the ice. Clean and sanitize and maintain in a clean and sanitary condition at all times. Cleaning is needed under, inside and behind food service equipment. Observed excessive food debris around the inside perimeter of the drawers of the bain marie. Conduct a full deep cleaning. Observed loose trash inside the dumpster corral. Clean up all loose trash and place into dumpsters - keep lids closed at all times. Maintain in a clean and sanitary condition at all times. Soap was not available at the handwash sink in the ladies restroom. Soap was provided at this hand sink during the inspection. Paper towel dispenser empty at the handwash sink in the front customer service area. Paper towels were provided during the inspection. Ensure that all hand sinks are properly stocked with paper towels and soap at all times. The light inside the walk-in freezer is not working. Replace light. The tile cove base is separating from the wall to the left and right of the rear exterior door. Make necessary repairs. Old unused equipment stored on the wire shelf near the rear exterior door, must be removed from food facility. Old toaster & espresso machine. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer. The CCHD has some concerns regarding the soft, mesh-like type of filters being used in the hood. Submit specifications for these filters for review. If these filters do not meet standards, then alternative, approved filters must be used. Clean the following surfaces: floor under pizza oven; floor in rear warewashing room; walls in rear warewashing room (under sinks & behind dishwasher). Complete the following repairs: reattached loose ceiling beam transition strip in pizza prep area; patch & paint damaged ceiling area in dining room; patch & paint damaged wall areas outside restrooms. Undercounter cooler located in sandwich prep area not holding time/temperature controlled for safety foods at or below 41°F as required (43°F). Utilize other cold holding until this unit is repaired. Repair by date. The handwash sink located in the Global Food area does not have water at a temperature of at least 85°F (63°F), correct immediately and utilize other hand wash sinks.

Sauce was not reheated to 165°F prior to be being placed in the hot holding unit. Sauce was reheated to 165°F during the inspection. Time in lieu of temperature being used in the food facility to control ready to eat Time/Temperature Controlled for Safety (TCS) foods (pizza). Logs on-site did not verify disposition of foods. Correct/update log system immediately. Logs onsite were adjusted.

Hands were then dried using a wiping cloth. Wiping cloth used for hand drying was left out on the cook line. A food employee was observed touching cooked chicken, a ready to eat food, with bare hands. Same employee used dirty apron as funnel/shoot to move cooked chicken into storage bowl. Re-train staff. Foods stored open with no covering in the freezers and 1 door cooler at cook line. Cover all foods today. Re-train staff. Raw chicken displayed next to raw pork/beef in common equipment with no barriers to prevent cross-contamination. Re-train staff. Several raw animal foods were stored above ready to eat foods in the walk-in cooler. Reorganize walk-in cooler with raw foods below ready to eat foods today. Organize raw foods by cooking temperatures today (highest at bottom). Re-train staff. Food ingredient storage containers (squeeze bottles), in the cook line area, are not labeled with the common name of the food. Label all bottles today. 1 large container of cooked chicken was held at 112°F, in the cook line area, rather than 135°F or above as required. Chicken was discarded due to temperature and bare hand contact. Cut broccoli was held at 47°F, in the cook line area, rather than 41°F or below as required. Re-train staff. Facility does not practice proper date marking. No date of preparation or date to discard is present on food containers throughout the facility. Provide a date marking system and label all food containers with the date of preparation or the date to discard that are being held for over 24 hours. Ensure all food products are discarded seven (7) days or less after being stored with the day of preparation counting as the first day. Comply today and every day. Prepackaged beverages (iced tea, lemonade) are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and nutritional facts. These items must be pulled from the consumer self-service cooler in the vestibule and either labeled with the above-noted information, or sold from behind the counter by a food handler/food employee. The handwash sink in the warewashing area was blocked by several items inside the sink and not accessible at all times for employee use. Items were removed during the inspection. Handwashing sinks must be easily accessible and supplied with soap and paper towels at all times. Re-train staff.

The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Cheese sauce in the steam table observed at 110°F. It was indicated that the sauce had been placed in the unit approximately 20 minutes prior. Cheese sauce was pulled to be reheated. Due to the repeat violations, facility must begin keeping reheating log. Shrimp observed thawing in standing water in the prep sink, which is not an approved thawing method. Fish observed thawing in reduced oxygen or vacuum packaging. These packages must be opened to allow oxygen to enter prior to beginning the thawing process. Fish was discarded on-site. Food debris and water accumulation was observed. Make necessary repairs. Observed employee personal belongings (cell phone & car keys) on table where waffles are prepared. Owner shall purchase and utilize handled scoops to dispense products. Cooked garlic in oil was left out at room temperature. This is a TCS food and must be kept cold (below 41°F) or hot (above 135°F). Product discarded on site. Owner makes the product fresh daily and will keep in the refrigerator moving forward. Several TCS foods in the walk-in cooler beared no date labeling. Keep all TCS foods prepared on site or transferred from manufacturer packing date labeled. If held in refrigeration use or discard products within 7 days. Observed a residential crock pot used for cooking/holding cooked rice with pork. Remove. Only commercial grade equipment should be utilized. Clean the walk-in cooler; soiled ceiling and floor. Maintain in a clean and sanitary state. Observed the following plumbing to be in disrepair: Drain line/catch tank for the walk-in condenser is leaking. Repair. The handwashing, mop and three compartment sink are slow to drain. Have the plumbing works serviced and repaired. Correct within 48 hours. Maintain plumbing works in good repair. The epoxy floor seal near the rear grease trap is deteriorated. Clean and apply a new epoxy floor sealant or approved flooring system. Correct within 30 days.

Test strips were available and used to fill sink with proper Cl concentration. Ensure staff use measuring device for bleach and test strips to ensure proper concentrations. Facility floor was dirty with food debris present. Amount of dirt was more than just "today's" dirt. Grout on floor may have lost it's glaze making it harder to remove dirt and debris. Department recommends a deck brush or other hard bristled broom for sweeping before mopping is done each day. Pay attention to floor/wall juncture areas where debris tends to accumulate.

Outdoor trash area: Cardboard is not being picked up and is piled on the ground. Department has received complaints of overflowing trash and worries of rats from this facility. It was learned that a new trash removal company has been contracted. Cans used to be stored in the shed outside but the new company will not remove them from the shed so cans are now stored outside. Cans are reportedly not always completely emptied leaving less room for new trash. Trash is currently removed twice per week on Mondays and Thursdays. Ensure that all trash and recycling are picked up each time removal occurs. If twice per week is not sufficient to keep trash cans from overflowing, then pick up must be increased. Department will monitor trash area and report deficiencies to Store Manager.
